MOTIVATE at Home is a new program to educate care partners of older adults living at home. We would like to learn from your experience using our site. You will answer a few questions now and another set of questions will be emailed to you within one week. In order to participate, you need to complete steps 1 - 4 as outlined below.

1. Register with your name and email address
2. Complete a brief survey when you register
3. Select the topic area you want to learn about and choose the following items within the selection.
   • Watch at least one video (approximately 30 minutes)
   • Listen to one audio feature or podcast (approximately 20-30 minutes)
   • View at least one downloadable poster (approximately 5 minutes)
4. Complete a second survey sent out via email in one week
